Type of a scam Phishing

I received a realistic envelope in the mail with the AT&T logo and marked “First Class Mail” with AT&T paid postage. The envelope included a letter with a fake account number saying that my auto pay has been canceled and that I can either re-enroll in auto pay online at att.com/autopay or I can fill out an included form with my credit card information and a voided check and send to P.O. Box 6265 Carol Stream, IL 60197-6265. I called AT&T and they had no record and said I was still enrolled in auto pay. They told me to bring the letter to an AT&T store, so I did. They confirmed it was a scam.
